Context
The development of data-processing techniques derived from the studies and theories provided by Complexity science in general — and by the theory of deterministic chaos, of dissipative systems and catastrophe theory in particular — has enabled the construction of intelligent systems for extracting information from heterogeneous databases (data mining and neural networks). Techniques for the graphical representation of this multidimensional information, in a suggestive and interactive manner, have allowed a better understanding of the interdependent processes that ensure the dynamic stability of a dissipative system in general, and have supported the development of new methods for diagnosing and monitoring patients’ health status in real time. Moreover, linking this information on-line within networks of medical institutions has allowed the accumulation of data sufficiently diverse to be interpreted within automated diagnostic systems based on artificial intelligence.
Studies on the assessment of a patient’s health status from the perspective of catastrophe theory and the study of the evolution of certain physiological parameters have allowed catastrophe theory to be applied through phase-space reconstruction of a model capable of objectifying domains of “normality”, pathological zones and, evidently, the boundary that separates the living state from the non-living state (the zone of comatose states).
The identification of such a space, of the specific operating domains and, obviously, of the limit states, can facilitate the monitoring of patients and suggest possible, specific treatments individualized to each person. A better understanding of the evolution between the normal and the pathological — and of a patient’s specific response to a given treatment, including the influence that certain bio-geophysical environmental factors may have on the course of the disease and on the efficacy of a treatment — may, over time, also allow the conception of techniques for resuscitating patients in limit states (comatose states).
Some of the applications resulting from this integrated study may be useful both for the development of the field of assisted diagnosis and for all telemedicine applications.
Objectives
– identifying the dimension of the phase space in which a functional model can be reconstructed, capable of allowing a discrimination between domains of normality and pathological domains;
– reconstructing the phase space and building a model of a patient’s “health status” from the on-line analysis of data provided by monitoring in the intensive-care unit;
– building an intelligent system of the neural-network type, with supervision and evolution, through heuristic programs (bio-monitoring of health status);
– developing methods for pre-processing the data acquired from patients under continuous monitoring in the intensive-care unit, with a view to making them compatible with the intelligent neural-network system with supervision and evolution, through heuristic programs;
– structuring a network of clinics in which the equipment of the intensive-care units is compatible with the developed intelligent bio-monitoring system, with a view to accumulating clinical cases capable of enhancing the effectiveness of the bio-monitoring system built.

Studii și cercetări privind monitorizarea continuă și resuscitarea pacienților aflați în stări comatoase prin utilizarea de metode și tehnici furnizate de știința complexității

Context
Elaborarea unor tehnici de procesare de date, derivate din studiile și teoriile furnizate de știința Complexității în general și de teoria haosului determinist, a sistemelor disipative și a teoriei catastrofelor în mod special, a permis realizarea unor sisteme inteligente de extragere de informații din baze eterogene de date (data mining și rețele neurale). Tehnicile de reprezentare grafică a acestor informații multidimensionale, într-o manieră sugestivă și interactivă, au permis o mai bună înțelegere a proceselor interdependente ce asigură stabilitatea dinamică a unui sistem disipativ în general și au asigurat dezvoltarea de noi metode de diagnoză și monitorizare în timp real a stării de sănătate a unor pacienți. În plus, relaționarea on-line a acestor informații în rețele de instituții medicale a permis acumularea unor date suficient de diverse pentru a putea fi interpretate în sisteme automate de diagnoză, bazate pe inteligență artificială.
Studiile legate de evaluarea stării de sănătate a unui pacient din perspectiva teoriei catastrofelor și al studiului evoluției unor parametri fiziologici au permis aplicarea teoriei catastrofelor prin reconstrucția în spațiul fazelor a unui model capabil să obiectiveze domenii de „normalitate”, zone patologice și evident frontiera ce delimitează starea de Viu de cea de neviu (zona stărilor comatoase).
Identificarea unui asemenea spațiu, identificarea domeniilor specifice de funcționare și evident a stărilor limită poate ușura monitorizarea unor pacienți și sugera tratamente posibile, specifice, particularizate la nivel de individ. O mai bună înțelegere a evoluției între normal și patologic, respectiv a răspunsului specific al unui bolnav la un anumit tratament, inclusiv a influenței pe care anumiți factori bio-geofizici ai mediului îi pot avea asupra evoluției bolii și a eficienței unui tratament, poate permite în timp și imaginarea unor tehnici de resuscitare a bolnavilor aflați în stări limită (stări comatoase).
O parte din aplicațiile rezultate din acest studiu integrat pot fi utile atât pentru dezvoltarea domeniului de diagnostic asistat, cât și pentru toate aplicațiile de tele-medicină.
Obiective
– identificarea dimensiunii spațiului fazelor în care se poate reconstrui un model funcțional, capabil să permită o discriminare între domenii de normalitate și domenii patologice;
– reconstrucția spațiului fazelor și realizarea unui model al „stării de sănătate” a unui pacient din analiza on-line a datelor furnizate prin monitorizarea în sala de terapie intensivă;
– realizarea unui sistem inteligent de tip rețea neurală cu supervizare și evoluție, prin programe de euristică (bio-monitorizare a stării de sănătate);
– elaborarea de metode de pre-procesare a datelor achiziționate de la pacienții aflați în monitorizare continuă în sala de terapie intensivă, în vederea compatibilizării lor cu sistemul inteligent de tip rețea neurală cu supervizare și evoluție, prin programe de euristică;
– structurarea unei rețele de clinici în care dotarea în cadrul sălilor de terapie intensivă este compatibilă cu sistemul inteligent de bio-monitorizare elaborat, în vederea acumulării de cazuistică, capabilă să dezvolte eficacitatea sistemului de bio-monitorizare realizat.
